Young Citizen 2010 Nomination Form

Nominations must be returned by Friday, 13 November 2009

Nomination Criteria

Aged from 14 to 20 years
Must have previously acted for the benefit of the borough
Be a good example to others
Live in the borough
Must not be related to any present or former councillor
Must not have received any award or nomination for the service which has prompted the current nomination

Nominees will be judged against the following criteria:

Service to family and/or the wider community
Number of nominations
Personal qualities - for example success in adversity, achieving excellence
Involvement in community / youth projects
Resident in the borough for the period of office
